TREATMENT RECOMMENDATION IMPLEMENTATION
Healthnet Assessments Inc has the team, clinicians and
clinic based programs across Ontario to deliver Independent Evaluation
recommendations in the claimant's home, community or at a clinic facility.
The home based or community based programs are designed
for implementation of personalized training sessions with a kinesiologist,
physiotherapist or chiropractor as requested. Independent Evaluation
recommendations that require clinic based programs such as a brief work
conditioning program or a comprehensive program such as chronic pain
rehabilitation is set up at CARF accredited facilities with the experience,
team and capability to deliver such programs. The programs are designed and
monitored by Healthnet Assessment case coordinator who is a clinician with 5
plus years experience in managing complex files.
Chronic Pain Rehabilitation Program:
This is an Interdisciplinary Chronic Pain program
structured around CARF standards and delivered at CARF accredited facilities.
It is a multimodal, interdisciplinary, functional restoration and cognitive
behavioural approach to pain management. The program also includes elements of
the Progressive Goal Attainment Program with focus on disability reduction
rather than solely pain reduction. The team comprises a medical director, CBT
therapist, team psychologist, kinesiologist and other clinical staff including
vocational expertise as required. It is a 32 day program with the objective of
reducing disability and pain and increase functional capability for claimants
who are beyond normal tissue healing despite previous attempt at
rehabilitation.
The program is designed for claimants who present with
limitations that interfere with their abilities to function physically,
psychologically and socially and or vocationally and who are at least three
months post accident. It excludes medically unstable claimants, claimants with
aggressive behaviour and those in psychological crisis.
Progressive Goal Attainment Program (PGAP)
Healthnet Assessment has clinicians with Progressive
Goal Attainment Program experience. PGAP is a program designed to minimize
disability by preventing maladaptive decline into sedentary patterns and
targeting psychological barriers to the rehabilitation process. This is
targeted to claimants who exhibit psychological risk factors for disability.
Our clinicians have incorporated PGAP principles in their disability management
and return to work coordination roles. The features adopted in such a program
are the maintenance of an activity log, activity scheduling, walking program,
increasing activity involvement and overcoming psychological obstacles.
Driving Rehabilitation Program:
This is a specialized recovery program addressing issues
of driving anxiety, fear, phobia and panic feelings in transportation
situations. The team in this program include Driving Therapist psychologist/CBT
therapist and Occupational therapist (For physical issues)
